How to apply for a treatment session
To be admitted for treatment, please fax or mail the following documentation to

1. Completed Registration Form with select preferred treatment session dates.
You can fill it online, download it in MS Word (registration-form.doc).
2. Recent (within 6 months) medical report by one of the patient's doctors stating the diagnosis and current condition of the patient.
3. Completed Medical Questionnaire
You can fill it online, download it in MS Word (questionaire.doc).
The documents will be reviewed by our medical staff and if there are no contraindications, we will make a reservation for the patient for the treatment dates requested. We will notify you by email or fax of the patient's acceptance into the treatment program and include the invoice with payment instructions.
At least one month prior to the treatment session, you must confirm your reservation and make payment.
